Creator authorization

The agency fills in this form, the creator signs it, then the agency signs. Fill it in on screen and save it as a PDF, or print it and write by hand. Nothing typed here is sent to GetMyDMCA. The agency keeps the signed form for at least 3 years after the last notice for this creator. If GetMyDMCA asks, the agency shows it within 7 days. GetMyDMCA keeps its emails about that check as its record and does not add the form to the agency's account.

What I authorize

I authorize the agency named above, and E.R MARKETING LLC (a Wyoming limited liability company operating the service as GetMyDMCA) acting at the agency's request, to do the following for the photos and videos I publish under the stage names above ("my content"):

  1. send copyright takedown notices under 17 U.S.C. section 512 and similar laws of other countries;
  2. file removal requests with search engines, websites, hosts, networks, domain registrars and platforms;
  3. sign them as my authorized agent, including the statement under penalty of perjury that they may act for me;
  4. receive and answer replies about them.

I confirm that

  1. I am 18 or older, and I publish under the stage names and profile links above.
  2. I own the copyright in my content, or hold an exclusive license to it.
  3. Everyone shown in my content was 18 or older when it was made.
  4. The details on this form are true.

What the notices show

Notices name me by a stage name and list my official profile links, not my legal name or contact details. Replies go to GetMyDMCA. A site may pass a notice to the person who posted the content, and Google may send copies to the Lumen database, which publishes them. GetMyDMCA verifies me before it sends anything. How long this lasts

This authorization lasts until I withdraw it in writing to the agency (an email is enough). The agency must then remove me from its GetMyDMCA account at once. GetMyDMCA then stops searching for me and cancels my takedowns not yet sent. Notices already sent stay sent. If the agency does not act, I can write to [email protected], and GetMyDMCA pauses my notices while it checks.
